Product Intro:
Erlonat 100mg is a targeted therapy manufactured by Natco Pharma Ltd. It is especially effective in treating cancers associated with EGFR mutations. It is used either alone or in combination therapy, depending on the type and stage of the cancer being treated.

Uses:
  1. Non-Small Cell Lung Cancer (NSCLC) with confirmed EGFR mutations
  2. Locally advanced or metastatic NSCLC in previously treated patients
  3. Pancreatic Cancer – used with gemcitabine
  4. Maintenance therapy post initial treatment response
  5. Recurrent or progressive solid tumors involving EGFR overexpression (investigational/off-label)
  6. First-line therapy in EGFR-mutant advanced NSCLC

Storage Instructions:
  1. Store at 15°C to 30°C
  2. Protect from moisture, heat, and direct sunlight
  3. Keep in the original blister pack
  4. Keep out of reach of children and pets

How it Works (Mechanism of Action):
Erlotinib selectively blocks the EGFR tyrosine kinase enzyme, which is involved in the signal transduction pathways regulating cell division and survival. By targeting this receptor, Erlotinib inhibits the proliferation of cancer cells that depend on EGFR signaling, particularly in EGFR-mutated cancers.

Side Effects:
Common Side Effects:
  1. Rash and dry skin
  2. Diarrhea
  3. Nausea
  4. Fatigue
  5. Anorexia
  6. Eye irritation
Severe Side Effects:
  1. Interstitial Lung Disease (ILD) – life-threatening, needs immediate attention
  2. Hepatotoxicity – monitor liver enzymes
  3. Gastrointestinal perforation – rare but serious
  4. Ocular toxicity – including corneal ulceration or keratitis
  5. Severe cutaneous reactions – such as Stevens-Johnson Syndrome

Dosage (Typical Recommended Dose):
  1. NSCLC: 150mg once daily
  2. Pancreatic cancer (with gemcitabine): 100mg once daily
  3. Dose adjustments may be needed for adverse reactions or liver impairment

Method of Administration:
  1. Take orally with water
  2. Take at least 1 hour before or 2 hours after food
  3. Do not chew, crush, or break the tablet
  4. Take at the same time daily

Precautions:
  1. Confirm EGFR mutation before starting treatment
  2. Monitor for skin reactions, liver function, and pulmonary symptoms
  3. Avoid smoking, as it reduces drug efficacy
  4. Use sunscreen to prevent photosensitivity reactions
  5. Caution in hepatic and renal impairment
  6. Inform your doctor if pregnant or breastfeeding

Drug Interactions:
  1. CYP3A4 inhibitors (e.g., ketoconazole) – may increase Erlotinib levels
  2. CYP3A4 inducers (e.g., rifampicin, phenytoin) – may reduce effectiveness
  3. Antacids/PPIs – may reduce absorption of Erlotinib
  4. Warfarin – increased risk of bleeding, monitor INR
  5. Smoking – induces metabolism, lowers drug levels significantly

Allergies:
  1. Avoid if allergic to Erlotinib or any inactive ingredients
  2. Seek immediate medical help if symptoms include:
    1. Hives, rash
    2. Breathing difficulty
    3. Swelling of lips, face, or throat
    4. Severe dizziness

Overdose Information:
  1. Symptoms: Severe rash, diarrhea, nausea, liver damage
  2. Action: Seek immediate medical attention
  3. Provide supportive care – there is no specific antidote

Missed Dose Instructions:
  1. Take the missed dose as soon as remembered
  2. If it's almost time for the next dose, skip the missed one
  3. Do not double the dose to make up for a missed one

Additional Notes:
  1. Patients should be educated on rash management and hydration
  2. Monitor liver and renal function tests regularly
  3. Smoking cessation is strongly advised for best results
  4. Avoid grapefruit or grapefruit juice, which may affect drug metabolism
