Product Intro:
Unigef 250mg contains Gefitinib, a tyrosine kinase inhibitor developed to treat certain forms of lung cancer. Manufactured by United Biotech, this medication is typically prescribed to patients with EGFR mutation-positive advanced or metastatic NSCLC. It offers a personalized approach to therapy, especially in patients who have not responded well to chemotherapy.

Uses:
  1. Treatment of advanced non-small cell lung cancer (NSCLC) with EGFR mutations
  2. First-line therapy in metastatic NSCLC
  3. Second-line treatment post-chemotherapy failure
  4. Maintenance therapy in EGFR-positive patients
  5. Management of locally advanced or recurrent NSCLC
  6. Under research for other EGFR-expressing malignancies

Storage Instructions:
  1. Store at 15°C to 30°C in a dry, cool place
  2. Protect from moisture, heat, and direct light
  3. Do not freeze
  4. Keep out of reach of children and pets

How it Works (Mechanism of Action):
Gefitinib is a selective inhibitor of the EGFR (epidermal growth factor receptor) tyrosine kinase.
  1. EGFR is often overactive or mutated in cancer cells
  2. Gefitinib blocks the EGFR signaling pathways by preventing ATP binding
  3. This suppresses cell division and tumor growth
  4. It is especially effective in tumors with activating EGFR mutations

Side Effects:
Common Side Effects:
  1. Rash (acne-like)
  2. Diarrhea
  3. Nausea
  4. Dry skin
  5. Loss of appetite
  6. Weakness or fatigue
Severe Side Effects:
  1. Interstitial lung disease (rare but serious)
  2. Liver enzyme elevation or liver dysfunction
  3. Severe skin reactions
  4. Eye problems (blurred vision, dry eyes)
  5. Gastrointestinal bleeding

Dosage (Typical Recommended Dose):
  1. 250 mg taken orally once daily
  2. No increase in dose is recommended due to lack of added benefit and increased side effects
  3. Continue therapy until disease progression or unacceptable toxicity

Method of Administration:
  1. Take orally once daily, at the same time each day
  2. Can be taken with or without food
  3. Swallow the tablet whole with water
  4. If the patient cannot swallow, the tablet can be dispersed in water and taken immediately

Precautions:
  1. Confirm EGFR mutation before starting therapy
  2. Avoid in patients with severe liver or lung impairment
  3. Not recommended during pregnancy or breastfeeding
  4. Use effective contraception during and for 2 weeks after the last dose
  5. Monitor liver function and lung health regularly

Drug Interactions:
  1. CYP3A4 inhibitors (e.g., erythromycin, ketoconazole) may increase Gefitinib levels
  2. CYP3A4 inducers (e.g., phenytoin, rifampicin) can reduce efficacy
  3. Proton pump inhibitors and antacids reduce absorption
  4. May interact with blood thinners (e.g., warfarin) – monitor INR
  5. Avoid herbal products unless cleared by a doctor

Allergies:
  1. Do not use in patients with known allergy to Gefitinib or formulation ingredients
  2. Signs include:
    1. Severe rash
    2. Swelling of the face/lips
    3. Difficulty breathing

Overdose Information:
  1. Symptoms: Severe rash, diarrhea, nausea, vomiting, or liver toxicity
  2. No specific antidote
  3. In case of overdose, provide symptomatic and supportive treatment
  4. Seek immediate medical attention

Missed Dose Instructions:
  1. Take the missed dose as soon as you remember on the same day
  2. If close to the next scheduled dose, skip the missed dose
  3. Do not double the dose

Additional Notes:
  1. Always perform EGFR mutation testing before use
  2. Use under the supervision of an oncologist
  3. Avoid sun exposure to minimize skin reactions
  4. Encourage hydration and skin care for rash management
  5. Routine scans and labs are essential to monitor therapy effectiveness and side effects
