Product Intro:
Relipoietin 10000 IU is a high-dose injectable formulation of Erythropoietin alfa, developed by Reliance Life Sciences. It is especially beneficial for patients requiring significant stimulation of red blood cell production, such as those undergoing dialysis, chemotherapy, or with advanced chronic diseases. This product helps reduce the need for blood transfusions and improves overall quality of life.

Uses:
  1. Anemia in Chronic Kidney Disease (CKD) – both dialysis and non-dialysis patients
  2. Chemotherapy-induced Anemia – in cancer patients
  3. Anemia due to HIV therapy – especially in patients on zidovudine
  4. Bone marrow disorders – such as myelodysplastic syndromes
  5. Preoperative use – to reduce need for blood transfusions in elective surgeries
  6. Anemia in critical illness or chronic inflammatory conditions

Storage Instructions:
  • Store at 2°C to 8°C (refrigerated)
  • Do not freeze
  • Keep away from direct sunlight
  • Let it reach room temperature before injecting
  • Do not shake the vial or syringe

How it Works (Mechanism of Action):
Erythropoietin alfa acts on the erythroid progenitor cells in the bone marrow by binding to specific receptors. This stimulation leads to the proliferation and differentiation of red blood cells, which helps in increasing hemoglobin and improving oxygen delivery throughout the body.

Side Effects:
Common Side Effects:
  • Injection site pain
  • Flu-like symptoms (fever, fatigue)
  • Headache
  • Nausea
  • Muscle aches
Severe Side Effects:
  • High blood pressure
  • Thromboembolic events (blood clots, stroke)
  • Seizures
  • Pure Red Cell Aplasia (PRCA) – rare but serious
  • Severe allergic reactions (e.g., anaphylaxis)

Dosage (Typical Recommended Dose):
  • Dialysis patients: 50–100 IU/kg 3 times per week (IV or SC)
  • Non-dialysis CKD patients: 75–150 IU/kg once or twice a week (SC preferred)
  • Cancer-related anemia: 150 IU/kg thrice weekly or 40,000 IU once weekly (SC)
  • Dosage is titrated based on individual response and hemoglobin levels

Method of Administration:
  • Administered subcutaneously (SC) or intravenously (IV)
  • To be used under the supervision of a healthcare provider
  • Rotate subcutaneous injection sites to avoid irritation
  • Inspect solution for particles or discoloration before use

Precautions:
  • Do not exceed hemoglobin levels above 12 g/dL
  • Monitor blood pressure, hemoglobin, and hematocrit regularly
  • Evaluate iron status; iron supplementation may be necessary
  • Use cautiously in patients with a history of cardiovascular disease, seizures, or thrombosis
  • Discontinue if signs of PRCA occur

Drug Interactions:
  • No major interactions known, but monitor closely if used with:
    • Iron supplements – often necessary to support red blood cell production
    • Antihypertensive drugs – as erythropoietin can increase blood pressure
    • Immunosuppressants – may alter bone marrow response

Allergies:
  • Contraindicated in patients with known hypersensitivity to erythropoietin or excipients
  • Allergic reactions may include:
    • Rash
    • Swelling of lips or throat
    • Breathing difficulty
    • Severe dizziness or fainting

Overdose Information:
  • Symptoms: Increased risk of hypertension, stroke, and blood clots
  • Management: Discontinue treatment and provide supportive care
  • Therapeutic phlebotomy may be considered if hemoglobin rises excessively

Missed Dose Instructions:
  • Administer the missed dose as soon as possible
  • If close to the next scheduled dose, skip the missed one
  • Never double the dose to make up for a missed dose

Additional Notes:
  • Treatment may take several weeks for full effect
  • Regular monitoring of hemoglobin, iron, and blood pressure is crucial
  • Inform your doctor of any unusual fatigue, shortness of breath, or dizziness
  • Smoking may reduce treatment efficacy
